Poor/lack of communication. I applied for Paid Family Leave through NYSIF. After being asked to provide new documents, I scrambled to gather and submit them, but I had no confirmation they would be accepted. Over the next two weeks, I called 7–8 times during business hours, left voicemails, and sent an email trying to confirm — but I never got a response. With no other way to get answers, I downloaded the NYSIF app on my own and only then discovered that my claim had been approved. When I tried to set up direct deposit, the app told me to call main number, and when I did, a different representative said a check had already been mailed out — which is why direct deposit wasn’t available. I was diagnosed with breast cancer in July fast forward after months of chemo I went in for a double mastectomy and diep flap over two months ago and we have no received ANY SORT OF PAYMENT. It took two weeks to just get a hold of the incompetent Dale and his supervisor Sue. I won’t use last names but know if you deal with him you won’t be able to get in touch with him. You’ll be told your paperwork first wasn’t sent over, then come to find out it was but was “lost in the shuffle”, then to be told a week later, “oh we actually need another form”, give them said form with our personal information, name CORRECT address all the things and they go and send 8 weeks to an address that we haven’t lived in in over a year and is NOT the

Dissatisfied and disappointed as well as with PEF as NYS DOT employee and getting into far accident on 3/26/2014. Never received anything.

Haven’t been paid in over two weeks from being out of work now I’m late with bills. This is so annoying cause it happens every time. It’s like they don’t care and don’t wanna pay you
