Centers Medicare & Medicaid

I am reaching out with all due respect to the centers of Medicaid and Medicare services by this method because I see no other. I have called their lines and have never been able to get a response I am a patient on Medicare and Medicaid and a former lvn (reference only). I have been experiencing episodes of unmitigated patient abuse, hostility , charting fraud, etc at a facility they pay on my behalf 100%. Further I have been retaliatory discharged from Azusa Dialysis center for the regulatory complaints to CDPH I made. The first was substantiated with the v452 attached below.
